Answer: In June 2013, twelve individuals representing various professions and organizations made the decision to organize a statewide initiative to improve advance care planning in the state. By December 2013, members of the North Dakota Advance Care Planning Initiative had drafted the organizations Vision, Goal, and Priorities. Vision To create a culture across North Dakota where continuous (on-going) advance care planning is the standard of care and every individuals informed preferences for care are documented and upheld. Goal To assist statewide community partners with the development and implementation of a comprehensive advance care planning program by December 2016. Four Priorities Four priorities were identified if a comprehensive advance care planning program was to be implemented by December 2016: * Identify the advance care model that was best suited for a rural state, * Organize as a nonprofit corporation, * Revisit the use of an advance care planning standardized medical order form, and * Establish a presence in North Dakota.
