Achievements In 2012, SETs Adult Services-Family Care Team was ranked 1st among all Care Management Units in Milwaukee County. SET received the first-ever Presidents Community Impact Award in 2011 from the Medical College of Wisconsin in recognition of the work SET does to improve the health of the community. SET received a Best Practice Award from the U.S. Department of Housing and Urban Development and the U.S. Department of Health and Human Services for services-enriched public housing. In 2003, SETs unique model was published as a case study of a high-performing partnership by the National Academy of Public Administration. 4,500 people, including 2,500 seniors and adults with disabilities, benefit from SETs evidence-based methods each year from 22 locations in Milwaukee: Grandparents: 2,500 senior and disabled adults maintain their health and age in their own homes exercising their own decision-making in Milwaukees 14 public housing facilities. Parents: 1,000 families and individuals are guided each year to establish and fulfill goals to increase resiliency in children and promote family stability. Children: 1,000 elementary school students develop life skills in emotional regulation, problem solving and decision making.
