We serve East Multnomah and North Clackamas counties. We are proud to be allowed in your home to care for your loved ones as we would our own. We offer a team approach that includes doctors, nurses, along with social workers, chaplain, volunteers and your primary care physician with patient care to assist the family through their journey. You have the right to choose your own hospice no matter what insurance you have. Hospice care is covered under the Medicare hospice benefit, Medicaid and most private insurances. What makes us different? We won't send the patient or family any additional bills if care is not 100% covered. What makes us different? We are a non-profit, small, community hospice organization, which allows our staff to serve 8 - 10 patients, while larger hospices sometimes see 18 - 20 patients. Our low nurse to patient ratio allows for more weekly visits and better, more personalized care. Hospice benefits those not only with terminal cancer but with late-stage kidney, lung and heart diseases, COPD, ALS, Parkinson's, Alzheimer's/dementia and more. The sooner a patient receives hospice care, the better their quality of life will be. Pain management is a top priority. Our services include continued bereavement support for the family after the patient passes for at least 13 months. Grief groups are open to anyone in the community no matter if they are affiliated with Mt. Hood Hospice.
