We believe the most powerful way to do this is collectively - by working together with service providers, elected officials, businesses, and local communities to end homelessness in the Capital region of Connecticut. We will build partnerships, advance innovative solutions, and achieve enduring, systemic change. As part of the national mayors Ten Year Plan movement, the City of Hartford convened the Commission to End Homelessness in the Capital Region in June 2004. One year later, the Ten-Year Plan to End Chronic Homelessness was completed. With input from more than 150 stakeholders from the public, private, and nonprofit sectors, the implementation plan for the Ten-Year Plan to End Chronic Homelessness was finalized in October 2007. To spearhead the implementation plan, Journey Home was launched as an independent, nonprofit organization with a mission to end homelessness in the Capital region. Journey Home is unique in that it uses a collective impact model as the engine for system change to transform the homeless services sector and impact the lives of the clients we all serve. Focus on collective impact over the traditional model of collaboration is intentional; it has shown its effectiveness in bringing together organizations and stakeholders within and across sectors to reach the common goal of ending homelessness in the Capital region. Collective impact models call for a backbone agency to guide vision and strategy, support aligned activities, establish shared measurement practices, build public will, advance policy, and mobilize funding. Journey Home serves as the backbone agency working to end homelessness in the Capital region.
