Our values endorse our belief that the people we care for are our highest priority. * Colonial Poplin is a skilled rehab facility that can support all of your respiratory needs. Our goal is to provide short term interdisciplinary rehabilitation for patients with a primary pulmonary or cardiac diagnosis and with acute conditions affecting their functional abilities. Patients are taught self-management strategies enabling them to reach their highest functional independence and prevent re-hospitalizations, often exceeding their prior level of function in a safe and effective manner. In addition to our comprehensive rehab, our cardio pulmonary program includes: Medical Director and Nurse Practitioner visits 4-5 times per week. On-sire Respiratory Therapist performing weekly rounds, Extensive resident and family education, Individualized treatment plans, Oxygen Therapy, C-PAP/BI-PAP, Aerosolized medication management, Registered Dietician, Secretion Mobilization & Airway Clearance, Chest tube care, Diagnostic testing. * Other services include: Physical Therapy, Occupational Therapy, Speech Therapy, and Advanced Wound Care Management including wound vac, Treatment of Joint replacements, Traditional skilled nursing care, Hospice Care, Sub-acute services including IV Therapy, Geriatric mental health services, Full activity program and bus trips, Long term and respite care, Full service cafe on-site, and Poplin Way Assisted Living Community.
